    Only umpires submit an Ejection form. Any one can submit a General incident form. All forms to be submitted within 48hrs of any incident or infraction. When in doubt contact the WOBA Umpire in Chief or WOBA VP.

    Umpire Ejection Form
    This form is to be used by umpires for ejections from WOBA league games & league tournaments, and OBA Select and OBA Rep games & tournaments. Please complete within 48 hours of the ejection. This form will be automatically submitted to Baseball Ontario. You should receive a copy of your submitted form.

    Incident/Complaint Report Form
    If the situation is not an ejection but an incident of another nature whereby a coach, player or spectator(s) behave inappropriately at an organizational baseball activity, there is a responsibility to report it. This includes use of liquor, profanity, physical or verbal abuse, fighting, deliberate attempt to injure and for all requests of police or security. OBA reporting has partnered with ALIAS. This is a reporting service for wrongdoings and inappropriate situations within the baseball environment, its activities and organizational events. The objective is to provide a safe reporting system. Folks can report anonymously, and if not requiring further investigation or escalation, the report is then redirected to the OBA Integrity officer for review.
