Every WOBA team that participates in WOBA league play must complete this form to acknowledge their decision and commitment to participating at the League's year-end championship tournament. Teams who do not submit a form by Noon on the day of the July BOD meeting will not participate in their respective year-end championship tournament. Teams who withdraw after the tournament schedule has been published are subject to fines as set out in WOBA policy.
