Team Declaration & Commitment for League Year-end Championship Tournament

Every WOBA team that participates in WOBA league play must complete this form to acknowledge their decision and commitment to participating at the League's year-end championship tournament. Teams who do not submit a form by Noon on the day of the July BOD meeting will not participate in their respective year-end championship tournament. Teams who withdraw after the tournament schedule has been published are subject to fines as set out in WOBA policy.

I understand that by submitting our team's commitment form for the yearend tournament, that we have up until the date that the schedule is published to withdraw our submission without penalty.

I understand that by withdrawing our team's commitment from the yearend tournament, once the schedule has been published, is a fine equal to the entry fee for the tournament and shall be paid directly to the host of the tournament.
