Insurance Information
Information on Baseball Ontario’s Insurance Policy can be found on Baseball Ontario’s website at www.baseballontario.com 

On the home page, click on the button “About Baseball Ontario”, “Risk Management” “Insurance Info and Docs”.  You will be able to find information on the following:

·       Incident Report Form

·       Sport Accident Claim Form

·       Baseball Ontario Insurance Brochure

·       Coverage Overview of Insurance Policy

·       Out of Province/Country Insurance Information

 

It is important to remember to report all injuries, accidents and incidents to the Baseball Ontario Office as soon as possible by submitting an INCIDENT REPORT.  Once we receive the Incident report, Baseball Ontario will inform Jones Brown Insurance of the incident.  Jones Brown Insurance Inc. must receive this notice of the accident date within 30 days from the date of the injury and they must receive the Sports Claim documentation within 90 days of the incident. 

 

Baseball Ontario strongly recommends that all Local Associations make everyone in their Association aware of these procedures.  It is important to report all incidents to Baseball Ontario.

 

All requests for and/or changes to “Certificates of Insurance” must be sent to the Baseball Ontario Office.  If you require a certificate of insurance, please forward your request to Baseball Ontario by email at  [email protected] 


If you require a certificate of insurance that needs a group to be added as additionally insured, we will need the official name of the company and address as well.
